Acute Renal Failure after Ingestion of Polyethylene Glycol Electrolyte Lavage Solution for Bowel Preparation in a Chronic Spinal Cord Injury Patient: A Case Report

چکیده
Intravenous pyelography (IVP) is a common tool for evaluating the upper urinary tract in spinal cord injury patients. To obtain a proper image, bowel preparation is required. Among the regimens for bowel preparation, polyethylene glycol (PEG) electrolyte lavage solution has a good cleansing quality and fewer side effects. Nonetheless, adverse effects of the regimen, though rare, have been reported. We describe a spinal cord injury patient who suffered from acute renal failure as a result of PEG-related toxicity, after taking oral PEG electrolyte lavage solution for bowel preparation. A 57-year-old woman who had sustained spinal cord injury for 11 years was admitted for orthopedic problems and the follow-up of urinary system. Her serum blood urea nitrogen, creatinine, and electrolytes were within normal ranges on admission. She took 500 mL of PEG electrolyte lavage solution for bowel preparation. Mild vomiting and skin rash were noted 30 min later. Moreover, oliguria developed within the next 20 h so she was diagnosed with acute renal failure. The presence of metabolic acidosis and urinary calcium oxylate crystals were noted, so PEG-related oliguric acute tubular necrosis was suspected. The above symptoms were similar to those seen with ethylene glycol intoxication. We conclude that bowel preparation with PEG lavage solution may be associated with severe renal complications, and that physicians should be aware of possible adverse effects when administering the agent. Bowel preparation with polyethylene glycol electrolyte lavage solution is potentially hazardous in patients with carcinoma of the cardia: a case report.
Polyethylene glycol (PEG) electrolyte solution can induce potentially fatal complications when used as a lavage in preoperative bowel preparation. A 60-year-old man with carcinoma of the esophagogastric junction had a Mallory-Weiss tear after bowel preparation with PEG. He was successfully resuscitated, first with balanced salt solution then by transfusion. The literature on the subject is revi...

Background: Ethylene glycol is a toxic alcohol which is used in brake fluid, antifreeze, coolants, preservatives and chemical solvents. Ethylene glycol poisoning usually results in depression of the central nervous system, renal insufficiency and cardiopulmonary compromise, while laboratory findings include metabolic acidosis, increased anion gap, increased osmolar gap and calcium oxalate cryst...
متن کاملAdverse renal and metabolic effects associated with oral sodium phosphate bowel preparation.
Colorectal cancer can be prevented by the removal of adenomatous polyps during screening colonoscopy, but adequate bowel preparation is required. Oral sodium phosphate (OSP), an effective bowel purgative, is available over the counter and requires a substantially lower volume than polyethylene glycol-based preparative agents. Accumulating reports implicate OSP in electrolyte disturbances as wel...

both very common and widely prescribed agents for bowel preparation and diabetes control, respectively. Renal damage after usage of OSP and lactic acidosis after metformin are both well recognized adverse effects. Hence, there are regulations for each of these two agents to limit their use in specific populations. However, the safety of OSP usage in patients taking metformin is seldom discussed...

Background: Colonoscopy is the preferred procedure for diagnosis of large-bowel diseases in dogs. Polyethylene glycol (PEG) is the most commonly used laxative for colonic cleansing. Senna has been used in traditional medicine as a laxative and an aid to treat constipation. OBJECTIVES: The aim of this study was to evaluate the safety of senna as an alternative or adjunct to polyethylene glycol s...
